Our Nurse Practitioner provides primary care services at some Assisted Living and Independent Living communities in LaPorte and Porter County IN so you don't have to leave your home for primary care visits.
The NP visits Assisted Living communities every 2 weeks for any routine visits, acute illnesses, wellness visits, preventive care, chronic diseases, home health certification, lab or imaging ordering, prescription management, orders for therapy, earwax removal, and more.
Chronic disease management for conditions such as heart failure, diabetes, high blood pressure, heart disease, dementia, depression, anxiety, COPD, high cholesterol, arthritis, a-fib, IBS, peripheral vascular disease, and more.
Acute treatment for acute illnesses like colds, flus, stomach upset, diarrhea, vomiting, constipation, bladder infection, sinus infection, ear infection, wounds, rash, fever, falls, weakness, dizziness, unsteadiness, pneumonia, weight loss, and more.
Once we are your established Primary care provider, our NP will work with the Assisted Living Nursing Team and you to address any medical concerns that arise in between NPs visits. One of our goals is to reduce or prevent urgent care, ER and hospital visits. We also want to help you meet your health care goals and needs in a convenient manner.

Our Nurse Practitioner, Jackie, is a board-certified adult gerontology primary care nurse practitioner licensed in the State of Indiana who works under a collaborating family physician licensed in Indiana.
Jackie has over 12 years experience as an NP plus 9 years as a registered nurse. She has experience in primary care in the adult geriatric population, wound care, neuropsychiatry, and home health. She has been seeing patients in skilled nursing and assisted living communities for over 8 years.
